Transition Services

Transition Services may be provided to adolescents, 15-17 years old, who have a diagnosis of High Functioning Autism or Asperger’s Syndrome.

Transition Services start in a structured group format and build skills that are most relevant to the individual as they approach adulthood, with an emphasis on social and life skills development. The intended outcome of the program is a generalization of skills to more natural settings; therefore supports are eventually transferred to community settings.

Transition Services may include:

  • Supported Employment Services
  • Resume writing
  • Transportation and job safety
  • Onsite support – job coaching
  • Co-worker/peer awareness training


Availability

Transitional Services are only available in the Sudbury District. Transition Services will be offered in the Thunder Bay district in 2013.

This service is only accessed through internal referral from your ABA Services Lead. Contact your lead for more information.

If you have a child who is not currently enrolled in ABA Services, learn about the intake process by contacting your local Intake Agency.
